Description & Responsibilities
Upham's Corner Health Committee, Inc./dba/Upham's Corner Health Center
The COVID-19 Nurse will work in a team environment and provide lead clinical support and participation in all COVID-19-related services including community testing and vaccination programs.
This is a grant-funded position during the COVID-19 pandemic with potential to become permanent as part of primary care nursing staff.
Duties & Responsibilities:
- Working in the COVID Testing Tent collecting specimens (e.g. - Nasopharyngeal swab) from asymptomatic and symptomatic people in an effort to confirm or rule-out the COVID- 19 virus.
- Ordering testing in EPIC under the direction of the care provider
- Collect specimen and labeling
- Responsible for Vaccinating patients
- Provide follow up care and Patient /Family education as is clinically indicated
- Tracking and coordination of vaccine supply
- Critically think and assess patients who may present ill.
- Participate in all COVID-19 related educational activities.
- The RN provides leadership through activities such as preceptor role, informal leadership roles, and quality improvement efforts.
- During face to face encounter or over telephone calls conduct comprehensive assessment of patient's health and psychosocial needs
- Work with the care team to facilitate services and provide optimum care to the patients
- Engage patients into services in order to prevent disease exacerbation, improve outcomes, increase patient engagement in self-care, community resources and minimize hospital and ER utilization.
- Plan effective and realistic goals prioritizing patient's needs to maximize quality and cost effective outcomes
- Facilitate communication and coordination within the members of the care team involving the patient in the decision making
- Demonstrates clinical competence, self-direction and sound judgment in the provision of ongoing clinical care coordination, monitoring and care management for patients.
- Utilize OCHIN-EPIC to document all aspects of patient care
- Maintains patient confidentiality at all times
- Performs other related duties as required
Qualifications
Minimum Basic Knowledge:
Licensed Registered Nurse (B.S.N. preferred) in Massachusetts
Experience & Qualifications:
- Two years of nursing experience, preferably in an ambulatory care or community health setting.
- Maintains BLS (Basic Life Support) for Health Care Providers from one of the following programs:
1. American Heart Associate
2. American Red Cross - Experience with EMR and related care management registries, databases and Information Technology/Computer Systems.
- Demonstrated ability to work as an integrated member of the care team
- Excellent communication and clinical judgment skills.
- Experience in culturally diverse setting preferred.
- Bilingual in Spanish, Portuguese or Vietnamese preferred but not required
Independent Action:
- Functions independently within scope of nursing practice, the Primary Care Payment Reform Initiative's requirements and health center policies and goals.
- Refer specific problems to supervisor when clarification of operating policies or procedures is required.
Supervisory Responsibility:
There are no supervisory responsibilities
