We are immediately hiring for a LVN – Behavioral Health in San Leandro, CA.

The LVN provides direct client care including medications and basic medical intervention and education, following standard nursing practices.



  • Graduation from an accredited school of licensed vocational nursing or psychiatric technology required.
  • Associate degree, preferred.


  • One (1) year service experience working as a nurse in a behavioral health field
  • One (1) year medication administration experience required.

License or Certification:

  • Valid California LVN or LPT license.
  • Current CPR certification.


Essential Duties

  • Provides direct nursing care.
  • Visually and audibly observes, documents in writing and reports orally or in writing the physical or mental condition to the Clinical Director and the Nurse Practitioner or Psychiatrist.
  • Orders, prepares, administers and documents the administration of medication for clients per physician’ s orders. Confirms delivery of all ordered medication from pharmacy on shift worked.
  • Provides basic medical care, including first aide and medical intervention within scope of licensure as necessary for each individual client.
  • Documents all services provided in a clear and concise manner within the Electronic Health Record within the timelines and framework established by Stars.
  • Prepares, presents and coordinates a verbal or written change of shift report.
  • Ensures compliance with legal, regulatory, and clinical policies and procedures regarding medical care.

Other Responsibilities

  • Provides consultation to Behavioral Health staff.
  • Facilitates relevant psycho-educational groups with clients.
  • Participates in the Agency’ s and SBHG’ s Total Quality Management (TQM) and Continuous Quality Improvement (CQI) systems which support compliance with Joint Commission and other accreditation standards as applicable.
  • Participates in TQM activities as requested including CQI subcommittees, probes, peer review, audit and/or tracer activities as appropriate.
  • This job description is not intended to be all-inclusive and the employee will also perform other reasonably related duties as assigned by the Clinical Director as needed.


  • Ability to interact and communicate both verbally and in writing with participants, families, schools, other staff, physicians, county agencies, other health care and social services providers and the public.
  • Ability to work independently and as a member of a treatment team.
  • Ability to develop strong relationships with other agencies and community partners.
  • Good organizational, writing, and time management skills.
  • Demonstrated understanding of psychiatric illness, trauma, and its treatment.
  • Ability to understand and work with participants from diverse cultural backgrounds in a positive manner.


  • Ability to physically perform Pro-ACT.
  • Ability to visually and audibly assess participants’ behavior and needs.
  • Ability to provide first aide.
  • Ability to stand and walk for long periods, up to 3 hours at a time.
  • Ability to drive within the community in personal vehicle.


  • Potential for eye and wrist strain from computer screen and minor lacerations from paper cuts.
  • Repetitive movements – writing and typing.
  • Interaction with potentially assaultive youth.
  • Blood and Body Fluid Contact (Category I).


  • Demonstrated ability to administer medications, including injections.
  • Demonstrated ability to engage clients and families.
  • Demonstrated ability to assess and diagnose physical health status.
  • Demonstrated ability to formulate and implement an effective nursing care plan.
  • Demonstrated understanding of psychiatric illness and its treatment as appropriate within Scope of Practice.
  • Demonstrated understanding of Company’ s Compliance Program and Code of Ethics.
  • Demonstrated understanding of laws and regulations affecting behavioral health clinical care.
  • Ability to perform clear and concise documentation including the use of EMR where applicable.
  • Demonstrated understanding of how to behave in a safe manner while seeing clients in the community or facility as applicable.
  • Demonstrated ability to recognize dangerous behavior or health status change by clients and intervene as appropriate.
